Mac HCFA CMS-1500 Forms

I was wondering if anyone might know of a good Word and/or iPages template for plain paper billing using the HCFA CMS-1500 format. I understand that many providers use integrated practice management solutions that provide that functionality, but I want to also have a way of generating plain paper claims independently.

Any ideas or suggestions would be appreciated...
