Although not specifically geared to the medical community, I've been using Daylite from Market Circle for the past several months after suffering from several other less-than-optimal CRM options. I just started using the server based version this past week as I opened my office and would offer the opinion that could very well be rolled into or more appropriately morphed into a PMS with a little creativity.
